| When your employee's Aqua Allowance is depleted, they pay a deductible of $500 (single) or $1,000 (family) for any covered medical expenses in or out-of-network. |
After the deductible is met, Aqua pays 80% and your employee pays 20% of the cost of in-network medical care. The out-of-pocket maximum is $2,500 (single) or $5,000 (family). |
Once the out-of-pocket maximum is met, Aqua pays 100% of your employee's covered expenses for the rest of the year. |
